Fall Sports Interest Form

Welcome new and returning families and thanks for your interest in athletics this school year! Please submit this form to document your child's interest in playing on a sports team this fall. In addition to this form, you will also need to fill out athletic participation forms and upload your child's medical documents to our new digital platform, Seamless Docs, found here: https://dcgov.seamlessdocs.com/f/c4a57hp5qjqy. These forms must be uploaded and certified by the Athletic Trainer before your child will be permitted to participate so please submit them ASAP! Note: we are no longer accepting or processing paper participation forms.

The official start date of fall sports is Monday, August 2nd, however, dates for tryouts have not been confirmed. Dates for tryouts for specific sports will be shared on the Deal website and in our weekly updates.
